Midway Neurological & Rehabilitation Center in Bridgeview, IL is seeking a Psychiatric Rehabilitation Services Coordinator (PRSC) to lead individualized rehab plans and coordinate care across disciplines to meet residents' emotional and social needs.

You will collaborate with psychiatrists, therapists, and social workers, perform assessments, set goals, document progress, and ensure safety while delivering compassionate, evidence-based care in a supportive team environment.

Qualificações

• Bachelor's or Master's degree in social work, psychology, counseling, or a related field.
• PRSC and/or social services experience preferred.
• Strong knowledge of mental health disorders, treatment modalities, and rehabilitation strategies.
• Must meet health assessment requirements and state facility policies.
• Ability to lift at least 35 pounds and endure extended periods of sitting, standing, and walking.

Responsabilidades

• Develop, implement, and coordinate psychiatric rehabilitation programs tailored to individual needs.
• Assess residents' psychiatric, social, and vocational needs.
• Collaborate with residents to set rehabilitation goals and monitor progress.
• Provide ongoing case management and support to residents in the program.
• Collaborate with psychiatrists, therapists, social workers, and other healthcare professionals for holistic care.
• Participate in interdisciplinary meetings to discuss cases and treatment plans.
• Prepare care plans with goals and discharge plans with the team.
• Maintain accurate records and document changes in treatment plans.
• Make rounds to ensure resident and staff safety.
• Stay updated on facility policies and crisis management procedures.
