Nexus at River Oaks is seeking a Psychiatric Rehabilitation Services Coordinator (PRSC) to help residents improve emotional well-being, coping skills, independence, and quality of life. You will work with residents, families, clinical staff, and community providers to support recovery-focused care and individualized goals.

Responsibilities include reviewing treatment plans, preparing residents for plan reviews, coordinating rehabilitation services, and quarterly re-evaluations.

Qualifications

• Higher education degree in a human service field (e.g. social work, psychology, sociology, counseling psychology).
• Demonstrated sensitivity and skills in working with residents, families, staff and community agencies.
• Ability to present concise, meaningful written reports and articulate residents' social/psychological needs.

Position Overview:

Coordinate mental health and psychosocial support services to help residents improve emotional well-being, coping skills, independence, and quality of life. This role works closely with residents, families, clinical staff, and community providers to support individualized behavioral health goals and recovery-focused care.

Available Schedules:

10am to 6:30pm Mon-Fri

12pm to 8:30pm Tues-Sat

Key Responsibilities:

• As a psychiatric rehabilitation services coordinator (PRSC), you will review and assist the resident in understanding the treatment plan.

• You will prepare and assist the residents with active participation in the treatment plan review.

• You will provide and/or coordinate the delivery of the psychiatric rehabilitation services programs.

• You will re-evaluate the residents on a quarterly basis.
