Qualifications

• Bachelor's or Master's degree in a human service field (Social Work, Psychology, Sociology, Counseling).
• At least 1 year of supervised experience in mental health or human services.
• Demonstrated skills in working with residents, families, staff and community agencies.

Responsibilities

• Provide residents with a stable therapeutic relationship.
• Welcome and orient new residents to the facility.
• Review and assist residents in understanding their treatment plan and program schedule.
• Coordinate the delivery of psychiatric rehabilitation services.
• Complete the resident's comprehensive assessment accurately and timely.
• Re-evaluate residents quarterly and document findings.
• Oversee the interdisciplinary team in developing the individual treatment plan.
• Document progress with regular notes and incidental/quarterly entries.
• Refer residents to community agencies as needed.
