Nexus Health is hiring a Psychiatric Rehabilitation Services Coordinator (PRSC) to support residents’ emotional well-being and recovery-focused care. The role collaborates with residents, families, clinical staff, and community providers to align treatment plans with individual goals.

Typical shifts run 10am–6:30pm Monday to Friday or 12pm–8:30pm Tuesday to Saturday, with a focus on coordinating services and ensuring active resident participation in care reviews.

Psychiatric Rehabilitation Services Coordinator (PRSC)

Position Overview:

Coordinate mental health and psychosocial support services to help residents improve emotional well-being, coping skills, independence, and quality of life. This role works closely with residents, families, clinical staff, and community providers to support individualized behavioral health goals and recovery-focused care.

Available Schedules:

10am to 6:30pm Mon-Fri

12pm to 8:30pm Tues-Sat

Key Responsibilities:

• As a psychiatric rehabilitation services coordinator (PRSC), you will review and assist the resident in understanding the treatment plan.

• You will prepare and assist the residents with active participation in the treatment plan review.

• You will provide and/or coordinate the delivery of the psychiatric rehabilitation services programs.

• You will re-evaluate the residents on a quarterly basis.

Preferred Qualifications:

• Higher Education degree in human service profession (i.e. social work, psychology, sociology, counseling psychology).

• Demonstrated sensitivity, ability and skills in working with and understanding the needs of the residents, family members, staff members and personnel from community agencies.

• The ability to present concise, meaningful written reports and articulate the social/psychological needs of the residents.
