Job summary

Midway Neurological & Rehabilitation Center is hiring a Psychiatric Rehabilitation Services Coordinator (PRSC) to coordinate and oversee psychiatric rehabilitation services for residents with mental health disorders. You'll work with an interdisciplinary team to develop and implement therapeutic services and ensure emotional and social needs are met.

Qualifications

• Bachelor's or Master's degree in social work, psychology, counseling, or a related field.
• PRSC and/or social services experience preferred.
• Strong knowledge of mental health disorders, treatment modalities, and rehabilitation strategies.
• Must be capable of performing essential functions of the job.
• Must have patience, tact, and enthusiasm to handle difficult situations.
• Must meet health assessment requirements and pass background checks as applicable.
• Ability to push/pull/move and lift up to 35 pounds; sit/stand/walk for extended periods.

Responsibilities

• Coordinate and oversee psychiatric rehabilitation programs for residents with mental health disorders.
• Conduct thorough assessments of residents' psychiatric, social, and vocational needs.
• Collaborate with residents to set rehabilitation goals and monitor progress.
• Provide ongoing case management and support to individuals in the program.
• Collaborate with psychiatrists, therapists, social workers, and other professionals for holistic care.
• Participate in interdisciplinary team meetings to discuss cases and treatment plans.
• Facilitate Care Plan conferences and document progress and discharge plans.
• Maintain accurate records and ensure updates to treatment plans.
