Job summary

Saba Health Care is seeking a Psychiatric Rehabilitation Services Coordinator to integrate, coordinate, and monitor residents' psychiatric rehabilitation services for a defined caseload. Responsibilities

• Coordinate and monitor residents' psychiatric rehabilitation services for a targeted caseload.
• Review and participate in treatment plans and care plan conferences with the interdisciplinary team.
• Provide 1:1 counseling, training, and behavioral interventions to develop resident skills.
• Prepare progress notes and ensure compliance with care plans and IDT recommendations.
• Assist in problem-solving social and practical needs and coordinate services and transportation.
