• Monitor resident progress and communicate changes or concerns to the team.
• Participate in care planning and interdisciplinary meetings as assigned.
• Complete required progress notes and documentation accurately and timely.
• Advocate for resident rights, dignity, and independence.
• Assist residents with accessing facility and community resources.
• Maintain resident privacy and confidentiality and follow policies.
