Qualificações

• Master's degree in human services or related field.
• Minimum 1 year of experience with residents, crisis intervention, documentation, care planning, and involving families.
• Knowledge of MDS, discharge planning, and problem-solving.
• Ability to work in an interdisciplinary team and maintain confidentiality with professional discretion.
• Strong communication and leadership skills.

Responsabilidades

• Coordinate services, programs, and data collection to ensure the highest quality of care for residents.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Provide crisis intervention, documentation, and care planning, involving families in the process.
• Oversee data collection and review of resident performance, ensuring regulatory compliance.
• Facilitate skills groups and psycho-social groups, and provide problem-solving support.
• Work in an interdisciplinary team environment with confidentiality and discretion.
