Psychiatric Rehabilitation Service Coordinator

As a Psychiatric Rehabilitation Service Coordinator, you will play a vital role in helping clients assess their situation, set goals, and obtain required services. You will establish a course of action by exploring options, setting goals with clients, and obtaining assistance for clients by referring them to community resources, arranging for appointments, and establishing rapport with other agencies.
Essential Duties and Responsibilities:
- Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs.
- Collaborate with other professionals to assess client needs or plan treatments.
- Advocate for clients or patients to resolve crises.
- Intervene in crisis situations to assist clients.
- Refer patients, clients, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement, or education.
- Refer individuals to educational or work programs.
- Refer clients to community or social service programs.
- Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
- Monitor, evaluate, and record client progress per measurable goals described in treatment and care plans.
- Maintain client records.
- Plan discharge from care facility to home or other care facility.
- Modify treatment plans to comply with changes in clients' status.
- Identify environmental impediments to client or patient progress through interviews and review of patient records.
- Collect information about clients.
- Interview clients to gather information about their backgrounds, needs, or progress.
- Plan programs to address community health issues.
- Complete documentation required by programs or regulations.
- Complete and file required record-keeping forms/charts upon the resident's admission, transfer, and/or discharge.
QUALIFICATIONS:
- Must have a Bachelors degree in Human Services
- Must be able to read, write, speak, and understand the English language.
- Must be able to make independent decisions when circumstances warrant such action.
- Must be knowledgeable of social services and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Experience in Long Term Care is preferred.
PHYSICAL DEMANDS:
- Be able to move intermittently throughout the workday.
- Must be able to cope with the mental and emotional stress of the position.
- Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
- May be necessary to assist in the evacuation of residents during emergency situations.
