Qualifications

• Bachelor's degree in psychology, social work, counseling, or related field.
• 1+ year of experience in a behavioral health or psychiatric setting, preferably in long-term care or SNF.
• Familiarity with state psychiatric rehabilitation regulations, PASRR Level II documentation, and individualized treatment planning.
• Strong communication, advocacy, and organizational skills.
• Empathy, cultural competence, and ability to work with diverse resident populations.

Responsibilities

• Assess the psychosocial and psychiatric rehabilitation needs of residents upon admission and regularly thereafter.
• Develop, implement, and monitor Individualized Recovery or Rehabilitation Plans (IRPs) in coordination with the interdisciplinary team.
• Coordinate services with psychologists, psychiatrists, case managers, social workers, and direct care staff.
• Facilitate or oversee therapeutic groups and mental health programming.
• Ensure documentation complies with CMS/state psychiatric rehab regulations (e.g., MDS Section Q, PASRR, treatment goals).
• Advocate for resident rights and dignity, especially regarding psychiatric and behavioral health needs.
• Serve as liaison with managed care organizations, community mental health services, and state agencies.
• Participate in discharge planning and community reintegration efforts when applicable.
