Job summary

ViaQuest, Inc. is actively seeking a Psychiatric Nurse Practitioner to support adults 18+ in long-term care settings.

You will conduct face-to-face assessments and ongoing visits, manage treatment plans, and collaborate with facility staff to deliver personalized mental health care. The ideal candidate holds a graduate NP degree with PMHNP certification, possesses experience in psychiatric/behavioral health, geriatric care, and is willing to travel within the service area.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• PMHNP license required.
• Experience in psychiatric, mental and behavioral health services.
• Long-term care and/or geriatric experience strongly preferred.
• Outpatient and tele-psychiatry experience a plus.
• Willingness to travel within assigned service area.

Responsibilities

• Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
• Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
• Educating and supporting caregivers and facility staff.
• Managing and adjusting psychiatric medications to promote stability and well-being.
