Job summary

ViaQuest, Inc. seeks a Psychiatric Nurse Practitioner to provide specialized psychiatric care for adults in long-term care settings. Join a team dedicated to delivering individualized, high-quality mental health services to residents 18+.

Travel within the assigned service area is expected as part of the role. The ideal candidate will be a Graduate NP with PMHNP licensure, experience in psychiatric/behavioral health, and preferably LTC/geriatric exposure.

Qualifications

• Must have graduated from an accredited NP program.
• PMHNP license required.
• Experience in psychiatric, mental, and behavioral health services.
• Long-term care/geriatric experience strongly preferred.
• Outpatient and tele-psychiatry experience a plus.
• Willingness to travel within assigned service area.

Responsibilities

• Perform face-to-face psychiatric assessments and visits for adults in long-term care facilities.
• Maintain current patient records documenting problems, plans, actions, goals, and changes.
• Educate and support caregivers and facility staff.
• Manage and adjust psychiatric medications to promote stability and well-being.

Psychiatric Nurse Practitioner – Long-Term Care (Adults 18+)
