Job summary

ViaQuest is seeking a Psychiatric Nurse Practitioner for long-term care facilities in the Woodlawn area. You will conduct face-to-face adult psychiatric assessments, maintain records, and collaborate with facility staff.

The ideal candidate is PMHNP licensed, a graduate of an accredited NP program, with geriatric/long-term care experience; outpatient/telepsychiatry experience is a plus, and travel within the service area is required.

Qualifications

• Graduate of an accredited Nurse Practitioner program (PMHNP preferred).
• PMHNP licensure required.
• Experience in psychiatric, mental, and behavioral health services.
• Long-term care and/or geriatric experience strongly preferred.

Responsibilities

• Perform face-to-face assessments and ongoing psychiatric visits for adults in long-term care facilities.
• Maintain current patient records documenting problems, plans, actions and changes.
• Educate and support caregivers and facility staff.
• Manage and adjust psychiatric medications to promote stability and well-being.

At ViaQuest we provide specialized, individualized psychiatric care to adults in long-term care settings. We serve residents ages 18 and above, many of whom live with complex mental health needs, chronic illness, or developmental disabilities. Our psychiatric nurse practitioners partner closely with each facility's long-term care healthcare team to deliver individualized, high-quality mental health care for residents.

Responsibilities may include:

• Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
• Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
• Educating and supporting caregivers and facility staff.
• Managing and adjusting psychiatric medications to promote stability and well-being.

Requirements for this position include:
