Job summary

ViaQuest, Inc. is seeking a Psychiatric Nurse Practitioner to provide specialized care for adults in long-term care settings. You will conduct assessments and visits, manage medications, and collaborate with facility healthcare teams to support residents 18 and older.

The role requires a Graduate NP degree and PMHNP license, with prior psychiatric/geriatric experience preferred. Travel within the service area is expected, and a comprehensive benefits package is offered.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• PMHNP license required.
• Experience in psychiatric, mental, and behavioral health services.
• Long-term care and/or geriatric experience strongly preferred.

Responsibilities

• Perform face-to-face assessments and psychiatric visits for adults in long-term care facilities.
• Maintain current patient records documenting problems, plans, actions and changes.
• Educate and support caregivers and facility staff.
• Manage and adjust psychiatric medications to promote stability.

We serve residents ages 18 and above, many of whom live with complex mental health needs, chronic illness, or developmental disabilities. Our psychiatric nurse practitioners partner closely with each facility’s long-term care healthcare team to deliver individualized, high-quality mental health care for residents.

Responsibilities may include:

• Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
• Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
• Educating and supporting caregivers and facility staff.
• Managing and adjusting psychiatric medications to promote stability and well-being.

Requirements for this position include:

• Graduate of an accredited Nurse Practitioner program.
• PMHNP license required.
• Experience in psychiatric, mental, and behavioral health services.
• Long-term care and/or geriatric experience strongly preferred.
• Outpatient and tele-psychiatry experience a plus.
• Willingness to travel within assigned service area.
