The Jewish Board is seeking a Psychiatric Nurse Practitioner to join a mobile ACT team delivering integrated behavioral and medical health services to adults in New York City communities. You will perform comprehensive assessments, diagnose, plan treatment, and collaborate with a multidisciplinary team to support recovery.

Responsibilities include psychotropic management, medication oversight, crisis intervention, and in-home/community visits with clients and families.

Qualifications

• Licensed as a Psychiatric Nurse Practitioner in New York.
• A current DEA registration for prescribing controlled substances.
• ANCC certification or pending application is required.
• NYS OPRA status approved or application pending required and a Health Commerce Account by employment onset.

Responsabilités

• Provide health/behavioral health assessment and treatment planning.
• Provide medication management and prescribe psychotropic medications as appropriate.
• Attend daily team meetings and weekly supervision.
• Provide on-call after-hours crisis intervention as needed.
• Administer LAI injections and coordinate injection calendar with RN.
• Order and review labs, prior authorizations, and follow up with pharmacies.
• Travel to community visits in NYC as required.
• Collaborate with the multi-disciplinary team for care planning.
