ACT Team Psychiatric Mental Health Nurse Practitioner

About the Opportunity

We are seeking a highly skilled and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our client’s Assertive Community Treatment (ACT) team in Brooklyn. The PMHNP will play a crucial role in providing comprehensive mental health services to individuals with severe mental illness in a community-based setting. As the ACT Team PMHNP, you will collaborate with a multidisciplinary team to deliver holistic care, promote recovery, and improve the overall well-­being of their clients.

The annual base salary range is $165,000 to $170,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

Job Responsibilities

• Conduct thorough psychiatric assessments, including mental status examinations and medical histories, to accurately diagnose mental health disorders
• Formulate comprehensive treatment plans that address clients’ individual needs and goals, ensuring a person-centered and recovery-oriented approach
• Prescribe and manage psychotropic medications, considering clients’ clinical status, potential side effects, and medication interactions
• Monitor clients’ responses to medications, adjust dosages as necessary, and educate clients about their prescribed medications
• Provide evidence-based psychotherapy, counseling, and crisis interventions to clients, promoting symptom reduction and skill development
• Implement various therapeutic modalities, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and motivational interviewing
• Deliver services primarily in community settings, including clients’ homes, shelters, and other community locations, to enhance accessibility and client engagement
• Develop strong relationships with community resources, such as housing agencies, vocational programs, and substance use treatment centers, to support clients’ broader needs

Job Requirements

• PMHNP License in New York required
• Strong diagnostic and assessment skills, along with expertise in psychopharmacology and psychotherapy modalities
• Excellent interpersonal and communication skills for effective collaboration within a multidisciplinary team and engaging with clients and their families
