Job summary

Center for Human Development (CHD) seeks a Psychiatric Coordination Clinician to support assessment, treatment planning, and evaluation for clients receiving psychiatric medication management. You will work with adults, children, and families who are seen by a psychiatry provider and do not require frequent outpatient therapy.

Responsibilities include creating and maintaining treatment plans, updating assessments, and coordinating care with providers.

Qualifications

• Master’s degree in Social Work, Mental Health Counseling, or related field.
• Massachusetts licensure OR actively working toward licensure.

Responsibilities

• Provide annual comprehensive assessments focusing on level of care needs.
• Engage clients in routine check-ins and update treatment plans.
• Submit referrals to supportive services and maintain documentation.
• Adhere to privacy regulations and safety reporting procedures.
