Job summary

PathForward is seeking a Permanent Supportive Housing Case Manager for the Arlington Mill residences in Arlington, VA. This full‑time role reports to the Vice President, Housing and helps eligible participants obtain and maintain stable housing.

The position requires on‑site work, collaboration with a life skills team, and full case management including intake, leasing, documentation, and HMIS data entry. A BSW or related degree is required.

Qualifications

• Minimum BSW or Bachelor’s degree in a related field.
• Experience with substance abuse, mental health issues and chronically homeless individuals is preferred.
• Experience working with youths who are experiencing homelessness is preferred.
• Must be a self-starter with strong communication and organizational skills.

Responsibilities

• Engage eligible participants and assist with intake referrals.
• Coordinate housing location and leasing processes with Housing Locator and landlords.
• Maintain weekly case notes, service plans, and HMIS data entry.
• Provide transportation to appointments as needed.
• Attend team meetings and liaise with service providers to ensure continuity of care.
