Qualifications

• High school diploma or equivalent required; associate degree preferred.
• 4+ years in healthcare documentation or related fields.
• Strong medical terminology knowledge.
• Experience working with providers and healthcare partners.
• Familiarity with payer documentation requirements and workflows.
• Excellent written and verbal communication; strong organization and attention to detail.
• Experience using EMR, CRM, and office software.

Responsibilities

• Review clinical documentation for completeness, accuracy, and payer compliance.
• Identify missing or inconsistent documentation and resolve barriers.
• Verify documentation supports payer medical necessity requirements.
• Maintain accurate documentation tracking throughout review process.
• Partner with physicians, nurses, and clinical staff to obtain required documentation.
• Communicate deficiencies and corrections clearly to providers and teams.
• Collaborate with Revenue Cycle, Compliance, and Sales to streamline workflows.
• Ensure HIPAA compliance in all documentation activities.
