Job summary

Full Circle Health Network seeks a Provider Success Manager to strengthen California's network of community-based providers. This primarily remote, exempt role focuses on onboarding, grant coordination and provider engagement to support CalAIM initiatives.

You will analyze performance, build partnerships with CBOs and health plans, and report on operational progress while advising leadership on risks and opportunities across California.

Qualifications

• Bachelor’s degree in Healthcare Administration, Business Administration, Public Administration, Human Services or related field, or equivalent combination of education and experience.
• Minimum three (3) years of progressively responsible experience supporting healthcare operations, provider relations, implementation, project management or program operations.
• Experience with CBOs, healthcare providers, managed care organizations or public-sector healthcare programs.
• Experience coordinating complex initiatives.
• Strong written, verbal, facilitation, presentation and relationship-management skills.
• Proficiency with Microsoft Office and virtual collaboration technologies.

Responsibilities

• Lead introductory meetings with CBOs.
• Coordinate provider onboarding, implementation planning and operational readiness.
• Monitor, analyze and report provider performance, operational metrics and implementation progress.
• Identify barriers and recommend solutions.
• Build relationships with CBOs, providers, managed care plans and external stakeholders.
• Partner with internal departments to resolve provider operational issues.
• Prepare reports, presentations, training materials and operational updates.
• Advise leadership regarding provider implementation, performance, emerging risks and strategic opportunities.
• Represent FCHN in meetings, collaborative partnerships, conferences and trainings.
• Support provider engagement, operational excellence, network growth and strategic priorities.
• Perform other related duties.
