Oceans Healthcare in Lafayette, LA is seeking a Provider Liaison to support administrative coordination for assigned providers. The role focuses on documentation, scheduling, and communication under physician direction, and serves as the primary contact for nursing homes.

Qualified candidates may have nursing licensure or comparable experience with strong organizational and communication skills. Responsibilities include charting readiness, facility coordination, and participation in QAPI

Formación

• RN/LPN/LVN license in state of practice preferred.
• At least one year of acute medical-surgical and/or acute psychiatric experience.
• In lieu of licensure, six years’ experience in a similar role.

Responsabilidades

• Provide consistent, timely support to all assigned providers and manage administrative, scheduling, documentation, and coordination responsibilities.
• Prepare provider schedules and ensure patient notes are available for charting prior to provider rounding.
• Submit prior authorization requests to third-party payers when required.
• Serve as the primary telephone contact for assigned nursing home facilities regarding scheduling and coordination.
• Coordinate PEC and GDR documentation for assigned providers.
• Maintain professional relationships with providers and facility staff.
• Conduct on-site visits to support relationship development and coordination with facility partners; avoid interference with duties.
• Participate in Quality Assessment and Performance Improvement Activities as assigned.
