P3 Health Partners seeks a Provider Operations Coordinator to manage operational support initiatives and lead special projects focused on HEDIS quality gaps, risk adjustment, and affiliate engagement. The role involves outreach to patients for preventive screenings and coordination with clinicians to improve care quality.

The position requires problem-solving, meticulous tracking, and a collaborative approach across departments.

Qualifications

• High school diploma/GED required, associate degree in related field or equivalent experience preferred.
• 2+ years experience in healthcare-related fields required, clinical health care related experience, managed care, or Health plan experience strongly preferred.
• Experience with Microsoft Word, Excel, Power Point, Outlook and general office equipment such as copier, fax machine, required.
• Experience in Electronic Health Records required.
• Medical Assistant Certification or experience as a Pharmacy Tech, Radiology Tech, Lab Tech or similar strongly preferred.
• Experience with data mining preferred.
• Medical terminology knowledge required, experience with CPT II codes preferred.
• Current CPR Certification preferred.

Responsibilities

• Build and maintain effective relationships with assigned Affiliate groups as liaison to P3 departments.
• Advise and support affiliate practices with quality gaps and conditional documentation via outreach, data mining, audits, claims surveillance, and education.
• Coordinate scheduling and outreach for patients, finding dates/times that meet patient needs.
• Collaborate with groups and departments to manage high/risk patients across services (hospital discharge, hospice, referrals, care management, etc.).
• Assist in meeting preparation and participate in meetings as requested.
• Conduct data entry and maintain documentation for quality initiatives.
• Educate patients and affiliates on preventative screenings and their purposes.
• Perform in-home HEDIS gap closures for select measures when appropriate.
• Other duties as assigned.
