Camino Real Community Services in Floresville, TX is seeking a Provider Coordinator to manage a waiver services caseload under supervision of the Community Support Supervisor. You will monitor service delivery, develop Implementation Plans, and review services with each individual monthly.

The role requires documenting provider records, collaborating with internal teams, training in self-help and pre-vocational skills, and ensuring timely billing and compliance.

Formación

• High school diploma or equivalent.
• 2 years experience in related field or an Associate's degree in social, behavioral, or human services.
• Valid driver's license with clear driving record.
