Job summary

Dayton Physicians Network is seeking an RN Navigator to act as an advocate and educator for prostate cancer patients. You will coordinate care, liaise with patients, families and providers, and contribute to the knowledge base across the network.

The role emphasizes CARES values and requires RN licensure, with preferred BSN and 3 years’ experience in patient care or navigation. Travel to other locations may be required and EHR documentation is essential.

Qualifications

• Active RN License in applicable states.
• Associate degree required; Bachelor’s preferred.
• 3 years’ experience in patient care or as patient navigator.

Responsibilities

• Coordinate patient care including scheduling, care transitions, communication and referrals.
• Assess patients’ needs across medical, psychosocial, and financial aspects and make referrals.
• Interface with other healthcare teams for referrals and services.
• Educate patients, families and caregivers about diagnosis, treatment options and survivorship.
• Document all patient coordination activities in EHR systems.
