Job summary

LE0015 U.S. Urology New Jersey Practice, LLC is seeking an Oncology Nurse Navigator (ONN) to collaborate with Urology and Radiation Oncology teams in planning, tracking, and monitoring patients with Castrate Resistant Prostate Cancer (CRPC) and Advanced Prostate Cancer (APC).

The ONN will coordinate referrals, resources, and education, while ensuring continuity of care and optimal outcomes. The role requires RN licensure, 3+ years of clinical experience (urology preferred), and strong

Qualifications

• Graduate of an accredited RN program.
• Current RN licensure required.
• Three years of clinical-setting experience – Urology preferred.
• Reasoning ability to solve practical problems and interpret instructions.
• Computer skills with knowledge of information systems.

Responsibilities

• Plan, facilitate, track and monitor CRPC/APC patients within the U.S. Urology New Jersey Practice network.
• Coordinate referrals and resources based on patient needs and insurance coverage.
• Support quality reporting, patient satisfaction, and treatment adherence metrics.
• Ensure patients receive education and medications as prescribed.
• Facilitate appointments and act as liaison between patients and care team.
• Identify barriers to care and intervene with physician discussion as needed.
