Job summary

United Urology Group is seeking a Medical Oncology Nurse Navigator to guide patients and families through cancer care. The MONN will educate, facilitate decisions, and connect patients with resources, working closely with scheduling, financial counseling, pharmacy, labs, social work, and clinical teams.

The role requires BSN, RN with oncology experience, and certification within a year. Compensation is daily/hourly with full benefits and supportive work-life balance.

Qualifications

• Bachelor’s prepared Registered Nurse (BSN) required.
• Oncology Certification (OCN) preferred; Nurse Navigator Certification required to be completed within first year of employment.
• Five (5) years work experience as a Registered Nurse in an oncology practice or clinic.
• Urology or surgical experience preferred but not essential.

Responsibilities

• Performs coordination of care during the full continuum of cancer care experience.
• Identifies potential and real gaps in care delivery; seeks opportunities for avoidance and/or closure.
• Prepares for, organizes, and participates in the Patient Care Conference (huddle) at regular intervals; communicates interventional outcomes to full care team in a timely manner.
• Performs assessment of patient risk stratification.
• Performs basic psychosocial assessments and provides emotional support; delegates and/or makes referrals for additional screening and support as needed
• Develop appropriate nursing diagnoses and corresponding action plans as needed.
