The Cooperative is seeking a Family Connects NJ - Program Support Specialist to join our team across Burlington, Atlantic/Cape May, Camden, Cumberland/Salem, and Gloucester counties. This full-time role offers a salary of $49,000–$51,000 with a robust benefits package.

Responsibilities include non-clinical support, information management for the FCNJ Nurse Supervisor and RNs, and outreach to enroll families.

Qualifications

• Good team player.

• Skilled in all Microsoft Office Products.
• Valid driver’s license, car, and current insurance policy.

Responsibilities

• Contribute to equity and continuous quality improvement aligned with the agency's mission, goals, and advancement.
• Cross-collaborate with staff, leaders, and board members to foster information sharing and collaboration.
• Routinely disseminate information, presentations, trainings, and learning opportunities for internal staff.
• Participate in staff development opportunities to increase skill development and program goals.
• Outreaches and enrolls families in the FCNJ program.
• Schedule FCNJ RN visits with clients as needed.
• Organize forms and maintain program materials and handouts.
• Make reminder/outreach calls for client visits as requested.
• Make post-visit calls with clients served as requested.
• Compile resources and participate in community outreach activities as delegated by the supervisor.
• Contribute to the team effort.
• Perform other duties as assigned.
