Job summary

The Cooperative is seeking a Program Manager for Family Connects NJ (FCNJ) to supervise program support staff and coordinate family enrollment in the FCNJ program. This role collaborates with hospital partners through bedside recruitment and with the FCNJ leadership team to advance program goals.

Responsibilities include supporting equity and quality improvement efforts, cross-collaboration with staff and boards, staff development, and data reporting to the Director of Nurse Home Visitation.

Qualifications

  • Bachelor’s degree in social work, counseling, nursing or related health field.
  • Proven leadership abilities and supervisory potential.
  • Strong organizational and problem-solving skills; prioritization ability.
  • Professional demeanor; effective engagement with diverse clients and professionals.
  • Excellent oral and written communication; ability to work independently and in a team.
  • Valid driver’s license, insurance, and access to a car.

Responsibilities

  • Supervise FCNJ Program Support Specialist team with input from leadership.
  • Coordinate enrollment of families in FCNJ program and hospital partnerships.
  • Manage monthly expenses, time sheets, and PTO approvals.
  • Conduct staff performance reviews with leadership input.
  • Coordinate orientation and staff education for new directives.
  • Prepare data and reports for Director of Nurse Home Visitation.
  • Represent FCNJ on committee meetings as directed.
  • Perform other duties as assigned.

General Summary:

The Program Manager, Family Connects NJ (FCNJ) supervises program support specialists and coordinates the enrollment of eligible families in the FCNJ program. Assist with hospital partnerships through bedside recruitment and multidisciplinary team referrals. Works in collaboration with FCNJ team leadership.

Essential Functions:
  • Contribute to The Cooperative’s commitment to equity and continuous quality improvement that align with the agency’s mission, goals, and advancement
  • Effectively cross-collaborate with agency staff, leaders, and board members to foster a comprehensive approach of information sharing, working committees, and collective representation
  • Routinely disseminate relevant information, presentations, trainings, and learning opportunities for internal program and agency staff
  • When applicable, will participate in relevant staff development opportunities (trainings, summits, conferences, etc.) to increase skill development, elevate program goals, and engage staff members with timely key learnings
  • Supervises FCNJ Program Support Specialist team, with input from the Director of Nurse Home Visitatio
  • Approves and signs off on monthly expense reports, bi-weekly time sheets, and PTO requests.
  • Completes staff performance reviews for the evaluation process with input from Director of Nurse Home Visitation.
  • Coordinates orientation for new FCNJ staff
  • Coordinates staff education, training and the implementation of new program directives, with the Director of Nurse Home Visitation
  • Conducts quality reviews including review of documentation
  • Compiles data for reports, completes reports and submits to the Director of Nurse Home Visitation
  • Represents FCNJ on The Cooperative and DCF NJ committees/meetings, as directed by Director of Nurse Home Visitation
  • Performs other duties as assigned
Requirements
  • Bachelor’s degree in social work, counseling, nursing or other health-related field required
  • Demonstrated leadership skills
  • Organizational skills, problem solving skills with ability to prioritize work requirements
  • Must present in a professional manner and be able to engage with a wide range of clients and professionals
  • Ability to work independently and as a team member with good leadership, oral and written communication skills
  • Valid driver’s license, current insurance policy, and car
Physical Requirements:
  • Must be able to see to read documents and hear well enough to communicate with others
  • Must be able to write, type on a computer, and use a telephone
  • Must be able to operate a motor vehicle
  • Must be able to lift up to 10 pounds
  • Additional requirements to be determined with input from Director and HR
