Qualifications
Ability to pass fingerprint background check, CORI, DPPC and driving background checks.
Valid driver’s license required.
Valid MAP certification preferred.
Responsibilities
Assist with hands-on direct care including personal hygiene and meal assistance.
Ensure health and safety of individuals.
Provide education, training, counseling, and physical assistance to achieve ISP goals.
Communicate with family and support networks.
Develop, write, and present annual ISPs.
Perform case management duties.
Provide transportation as needed.
Schedule and supervise staff.
Skills
Experience working with individuals with ID/DD
Excellent verbal and written communication skills
Education
Bachelor’s degree or associate degree in a related field with two years of experience
HS diploma and five years of related work experience in human services
