Supportive Concepts for Families, Inc. is seeking a Program Specialist in Lewistown, PA. The role supports individuals with developmental disabilities, maintains caseloads, participates in planning, and coordinates services with families and providers.

We offer paid training, a $2,000 sign-on bonus, DailyPay, and a mission-driven team dedicated to meaningful work and client-centered planning. This full-time position emphasizes collaboration and quality services across residential settings.

Qualifications

• Associate's Degree (or 60 credit hours) and four (4) years of experience working directly with individuals with developmental disabilities
• Excellent communication, documentation, and organizational skills
• Detail-oriented with strong problem-solving abilities
• Valid driver's license, reliable transportation, vehicle registration and insurance
• Proficiency with Microsoft Office and electronic health record systems
• Ability to work independently and collaboratively with interdisciplinary teams

Responsabilités

• Maintain a caseload of individuals receiving residential services and ensure person-centered supports are implemented.
• Complete assessments, quarterly reports, annual reviews, outcomes documentation, and other required records.
• Attend ISP meetings, develop annual outcomes and support plans, and advocate for individuals' needs and goals.
• Serve as liaison between individuals, families, Supports Coordinators, community providers, and agency staff.
• Monitor residential programs, ensure compliance with standards, participate in QA activities, and support positive outcomes.
