Supportive Concepts for Families, Inc. in Lewistown, PA seeks a Program Specialist to provide person-centered supports within residential services. You will maintain a caseload, complete assessments, and coordinate services with families, coordinators, and providers.

Qualifications

• Associate's Degree (or 60 credit hours) with four years' experience in developmental disabilities.
• Excellent communication, documentation, and organizational skills.
• Valid driver's license, reliable transportation, current vehicle registration and insurance.
• Proficiency with Microsoft Office and electronic health record systems.

Responsibilities

• Maintain a caseload of individuals in residential services with person-centered supports.
• Complete assessments, quarterly reports, annual reviews, and outcomes documentation.
• Attend ISP meetings, develop annual outcomes and support plans, and advocate for individuals' goals.
• Serve as liaison between individuals, families, Supports Coordinators, providers, and staff to coordinate services.
• Monitor programs for compliance and quality, and collaborate with interdisciplinary teams.
