Job summary

COMHAR is seeking a skilled Program Specialist to coordinate and implement Individual Support Plans (ISPs) for individuals with intellectual, developmental, and behavioral health needs in the Philadelphia region. You will work with participants, families, and staff to ensure services are delivered effectively, promote independence, and support dignity and community integration.

Qualifications

• Associates or 60 college credits with at least 3 years related experience in ID required.
• Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred.
• Current PA Driver's License; satisfactory driving record; eligible to operate program vehicles.
• Experience with individuals with intellectual and developmental disabilities, mental health challenges, or autism.
• Knowledge of ISP development, regulatory requirements, and consumer-centered service delivery.
• Strong organizational, communication, and interpersonal skills.
• Able to maintain confidentiality and privacy of consumer records.
• Able to acquire working knowledge of COMHAR policies and regulations.

Responsibilities

• Coordinate initial, annual, and revision assessments for assigned caseload.
• Identify individualized consumer support needs for direct supervision or unsupervised periods.
• Disseminate assessments to ISP team members before ISP meetings.
• Provide training to DSPs on ISP components and medication protocols.
• Participate in ISP development, updates, and revisions.
• Attend ISP meetings and serve as Plan Lead when needed.
• Review ISPs and report discrepancies to the team.
• Implement ISPs, supervise services, and propose revisions as needed.
• Document monthly progress toward ISP outcomes.
• Report changes in consumer needs to the Supports Coordinator and team.
• Disseminate quarterly ISP reviews and inform team of options.
• Coordinate services and train DSPs on health, safety, and program needs.
• Develop components supporting life skills, employment, or goals.
• Ensure DPW 6400 compliance for medication management and training.
• Maintain accurate consumer records in HIRS.
• Monitor EIM incidents and ensure resolution per regulations.
• Support the behavioral component of ISP with Human Rights Team processes.
