Job summary

Easterseals NH in Concord, NH is seeking a Program Specialist to join our Community Based Services team. You will work with the Program Manager to develop and monitor goals for residents in a 3-woman home in Concord.

You will train Direct Support Associates, document progress, and escalate concerns as needed. The role includes direct care tasks such as personal care and transportation, with a 40-hour work week and weekend flexibility.

Qualifications

  • 1 year of Direct Support experience or 2 years with a HS diploma
  • Ability to provide on-the-job training and orientation to new DSAs
  • Valid driver’s license and reliable vehicle for transportation requirements

Responsibilities

  • Assist Program Manager with reviewing, monitoring and developing program goals
  • Act as Peer Guide for assigned DSAs, including on-the-job training and orientation
  • Maintain contact with DSAs for at least six months and report feedback to supervisor
  • Assist with preparation and compliance of required documentation
  • Provide direct support including personal care and transportation as needed

Job Description:Easterseals NH is looking for a Program Specialist to join our Community Based Services team! This program supports individuals within our residential homes.Responsibilities: Work directly with the Program Manager to assist in reviewing, monitoring and participating in the development, implementation and monitoring of program goals. Act as a Peer Guide for assigned new Direct Support Associates (DSA) which includes providing on the job training and program orientation with assigned DSA maintaining and documenting contact with assigned DSA for a minimum of six months, consistently demonstrate professionalism, exceptional customer service, and positive demeanor and elevate any positive feedback and/or concerns to supervisor. In this role you will also assist with preparation and compliance of required documentation as requested by supervisor and perform direct support responsibilities as necessary including personal care and transportation.About the home: This position is based in a residential home with 3 adult women in Concord.Qualifications: Associates Degree in Human Services or related field and (1) year of Direct Support experience OR a High School Diploma or equivalent and two years of Direct Support experience. You must have a valid driver's license, a reliable vehicle, and automobile insurance for transportation requirements.Hours: 40 hours/week.**Must be flexible, including weekends, to support program need.**Compensation: $25 HourlyWhat’s in it for you?
