Qualifications

• Minimum 8 years of experience with tobacco systems, tobacco prevention reporting/information systems, or community activity tracking systems.

• Minimum 3 years with tobacco cessation initiatives and systems.
• Minimum 25 years collaborating with healthcare providers (hospitals, FQHCs, clinics, behavioral health, insurers, provider groups, accountable care orgs).
• Minimum 10 years working with health plans/providers.
• Minimum 25 years with health system types and health systems change.
• Minimum 20 years coordinating with local, state, and federal organizations for systems change.
• Minimum 20 years facilitating systems change activities and improvement efforts.
• Minimum 10 years identifying trends in national/state health policy initiatives.
• Minimum 10 years in strategic planning.

• Contingent Upon Award

The selected Contractor must possess the following qualifications and experience:
Tobacco Systems & Community Tracking

• Minimum 8 years of experience working with the Department’s tobacco systems, tobacco prevention reporting/information systems, or community activity tracking systems.
• Minimum 3 years of experience working with the Department’s cessation tracking systems, including Tobacco Free Florida AHEC Cessation Tracker (TFFACT) and Florida Clean Air Act (FCAA) system.
• Minimum 3 years of experience working with tobacco cessation initiatives and systems.

Healthcare Industry Collaboration

• Minimum 25 years of experience collaborating with healthcare providers, including hospitals, Federally Qualified Health Centers (FQHCs), free and charitable clinics, behavioral health and substance abuse treatment centers, health insurance plans, provider groups, physician organizations, and accountable care organizations.
• Minimum 10 years of experience working with health plans and health plan providers.
• Minimum 25 years of experience working with health system types and principles of health systems change.

Systems Change & Strategic Initiatives

• Minimum 20 years of experience coordinating with local, state, and federal organizations to facilitate systems change within health systems.
• Minimum 20 years of experience facilitating systems change activities and improvement efforts.
• Minimum 10 years of experience identifying and documenting trends related to national and state health policy initiatives.
• Minimum 10 years of experience in strategic planning.

Technology & Process Improvement

• Minimum 10 years of experience working with health systems, including coordination, facilitation, and implementation of information technology solutions.
• Minimum 15 years of experience with technology systems and solutions related to healthcare systems, including cessation programs.
• Minimum 15 years of experience with program development and process improvement initiatives.
• Minimum 10 years of experience facilitating training efforts related to health systems change.

Financial, Evaluation & Leadership Experience

• Minimum 10 years of experience with financial management related to healthcare systems.
• Minimum 3 years of experience using healthcare and cessation evaluation findings to support systems change efforts.
• Minimum 15 years of experience in partnership building and relationship management within the healthcare field.
• Minimum 10 years of experience leading enterprise-wide teams (large and small) in healthcare environments.
• Minimum 10 years of project and program management experience focused on healthcare environments and health systems change.
