Job summary

Tierra del Sol Foundation is seeking a dedicated Program Manager in Sunland, CA to lead a team of Direct Support Professionals and align services with the organization’s goals. The role emphasizes collaboration with families, partners, and internal teams to deliver high-quality, person-centered supports.

The Program Manager oversees life planning, documentation, QA, transportation, and staff development, ensuring compliance and safe, meaningful community inclusion for associates.

Qualifications

• Experience providing training, instruction, and case management to adults with developmental disabilities is required.
• Experience implementing positive behavior support techniques is required.
• Experience in staff management, supervision, or leadership is essential.
• Strong organizational, written, and verbal communication skills.
• Ability to collaborate effectively with staff, families, community partners, and other service providers.
• Commitment to person-centered planning, community inclusion, and meaningful outcomes for associates.
• A bachelor’s degree in behavioral science or related field is preferred.
• Spanish proficiency is preferred.
• Current and valid California driver's license, at least three years of driving experience, and current auto liability insurance.
• Current CPR/First Aid certification.

Responsibilities

• Lead and support Productive and Personally Meaningful Life (PPML) Plans and person-centered planning, including PATH planning.
• Coordinate referrals, interviews, observations, and assessments to determine service options.
• Monitor progress toward individual goals using assessments and information from Direct Support Professionals.
• Coordinate case management, crisis support, and communication with families and partners.
• Prepare progress summaries, transition plans, and exit summaries.
• Develop and implement individualized behavior support plans as needed.
• Ensure families receive regular updates regarding progress and goals.
