Job summary

Pony Bird Inc. in De Soto, Missouri is seeking a qualified Program Manager to coordinate and deliver care for individuals with disabilities. You will develop service plans, oversee a 24/7 group home with rotating on-call duties, and supervise staff to ensure quality care in a supportive environment.

Qualifications

• 2+ years' experience in Program Management in a similar setting.
• Proficiency with MS Office and relevant software.
• BS in Social Work, Social Services, Nursing, Special Education, or related field.

Responsibilities

• Develop and implement Individual Service Plans, based on assessments and observations.
• Responsible for 24 hour a day Group Home with rotating on-call and scheduling.
• Supervise, plan, assign, and evaluate or monitor programming and the work of staff responsible for the daily care of individuals.
• Work closely with therapists, service coordinator, administrations, and parents/guardians to maintain communication and provide feedback, standardized procedures, expedite support plan implementation and work-flow, and to improve the efficiency of all care/support providers.
• Performs various assessments; chart reviews; documentation, mealtime and environmental monitoring.
