Job summary

Pony Bird, Inc. in DeSoto, Missouri is seeking a qualified Program Manager to coordinate care for individuals with disabilities in a 24-hour group home setting.

You will lead planning, assessments, and collaboration with therapists, service coordinators, families, and staff. The role requires a BS in a related field, at least two years in program management, proficiency with MS Office, and a Missouri driver's license with insurable record.

Qualifications

• BS in Social Work, Social Services, Nursing, Special Education, or related field from an accredited four-year college or university.
• 2+ years' experience in Program Management in a similar setting.
• Proficient in MS Office and various industry software programs.
• Valid Missouri License with an insurable driving record.

Responsibilities

• Develop and implement Individual Service Plans based on assessments and observations.
• Responsible for 24 hour a day Group Home with rotating on-call and scheduling.
• Supervise, plan, assign, and evaluate or monitor programming and the work of staff responsible for the daily care of individuals.
• Work closely with therapists, service coordinator, administrations, and parents/guardians to maintain communication and provide feedback, standardized procedures, expedite support plan implementation and work-flow, and to improve the efficiency of all care/support providers.
• Performs various assessments; chart reviews; documentation, mealtime and environmental monitoring.
