Job Location: Main Office - Denver, CO 80231, Position Type: Full Time, Salary Range: $22.36 - $26.61 Hourly, Position Purpose
This is a part time position requiring 20 hours per week
Under the supervision of the CMA Admin Supervisor, the Case Manager (CM) I provides support to Case Manager IIs, clients, community members, and external stakeholders. This position completes required case management activities and administrative tasks including, but not limited to, answering live phone calls in the Contact Center and calling clients to obtain and track for required documents. This position has the knowledge and tools to answer questions and problem solve for clients and stakeholders who contact RMHS. This role completes supports by identifying, researching, documenting, and resolving the appropriate level of support tasks that are brought up on a day-to-day basis. The support this position provides is imperative to ensure members receive critical access to services, and case managers and stakeholders have the information needed for continuation of services.
Essential Duties
- Gathers/thoroughly reviews/tracks/enters documentation in various data systems
- Communicates with clients, community members, medical professionals, internal case managers, and external stakeholders.
- Provides general guidance on LTSS services and processes.
- Maintains a professional, customer-service focus when working with internal and external stakeholders, upholding strict confidentiality and ethical standards.
- Reviews documents and completes tasks as assigned, and brings critical information to the attention of the case manager or supervisor.
- Enters log notes into internal data systems or the state-operated system and documents case management activities as it relates to each task.
- Provides timely, person-centered follow-up on all related duties with case managers, supervisors, managers, people accepting services, and supporting entities as necessary.
- Maintains accurate and up-to-date tracking for work completed.
- Responsible for accurate and timely completion of all tasks, reports, and documentation of case management activities.
- Assists with case management projects and other duties as assigned by supervisor.
- Participates in training and staff development opportunities. Actively participates in team meetings, and communicates progress and barriers with Supervisor and/or Program Manager.

- Basic skill in computer software programs including databases and on-line applications
- Ability to analyze information and use sound judgment, problem solve, and use critical thinking skills when making recommendations or solving problems
- Excellent written and verbal communication skills with good interpersonal skills.
- Ability to obtain training on required case management functions as directed by HCPF
- Strong organizational and time management skills, and the ability to handle multiple priorities with tight deadlines
- Ability to maintain strict HIPPA confidentiality guidelines
- Ability to function as a team player and work cross departmentally

Minimum Qualifications
- A bachelor’s degree; or
- Five (5) years of relevant experience in the field of LTSS, which includes Developmental Disabilities; or
- Some combination of education and relevant experience appropriate to the requirements of the position.
Preferred Qualifications
- Experience with Medicaid or human service programs
- Prior Case Management Experience
- Experience in a call/contact center
