Job summary

The Coordinating Center employs around 300 highly qualified coworkers, including Registered Nurses, Licensed Social Workers, Support Planners, Service Coordinators, and many more with vast experience in community-based care coordination.

Role Responsibilities
  • Collaborate with clients and their support team to develop individualized, person‑centered care plans that outline necessary services.
  • Conduct site visits to clients’ homes, schools, nursing facilities or other community settings to assess needs and monitor progress.
  • Coordinate services including locating new providers, accessing equipment and supplies, and scheduling transportation to medical appointments.
  • Maintain accurate documentation, meet program eligibility requirements, and adhere to all regulatory and compliance standards.
  • Facilitate client self‑advocacy and goal‑setting, ensuring they receive the resources and support needed to remain healthy and safe.
  • Participate in committees, team‑building activities, training sessions, and volunteer events to strengthen team culture and community engagement.
  • Manage work‑life integration by working from home with flexible scheduling options while balancing documentation and client care responsibilities.
