Job summary

The Coordinating Center is hiring Supports Planners to provide home-based care coordination for clients with complex medical and behavioral health needs across Maryland. You will collaborate with clients, families, and providers to create person-centered goals and connect them with community resources.

You will conduct assessments, develop service plans, and monitor progress while visiting client homes and coordinating care by phone or in person.

Qualifications

  • At least one year of experience in case management/coordinating services for individuals with complex medical needs and/or behavioral health needs of all ages is required.
  • Bachelor’s degree in human service focus (psychology, social work, public health, etc.) is strongly preferred.
  • Experience working with Medicaid and community resources is strongly preferred.

Responsibilities

  • Utilize assessments and information from various sources regarding medical, psychological, and social factors to evaluate and advocate for client needs.
  • Develop an individualized person-centered plan of service for each client, recommending supports and services.
  • Use critical thinking skills to troubleshoot conflicts or barriers within the support team including service issues and client crises.
  • Perform quarterly on-site visits to homes, nursing facilities, and community locations and monthly telephonic care coordination.
  • Locate appropriate resources for clients and work collaboratively with community agencies and providers to achieve desired client outcomes.
  • Assist individuals with developing life goals and monitors progress to ensure health and safety in the community.
  • Obtain and monitor estimates from various service providers including medical equipment and supplies.
  • Adhere to contractual billing guidelines when documenting all activities and tasks, including entering case notes and other documentation in multiple electronic database systems.
  • Assist in client transitions from medical facilities to a community setting.
  • Work collaboratively with our Nurse and Social Work consultants to resolve issues for those with complex medical and/or psychosocial needs.
  • Prioritize, develop, and manage your own schedule to ensure timely completion of work.

This is a full-time teleworking position with the expectation of visiting clients in the Baltimore region in Maryland.

What You Will Do:

Our case managers, who we call Supports Planners, play a significant role in the lives in individuals with complex medical needs, behavioral health needs, and/or those with disabilities. In this role you are at the center of coordination and planning while working collaboratively with your clients, their circle of support, and service providers within the community. Our clients value their partnership with our Supports Planners as they collaborate on the development of person-centered goals, and gain access to the best services and community resources to support their independence in their local community. This position is a home-based position, and requires visits to client homes, assisted living facilities, and other service provider locations within Maryland.

As a Supports Planner, you will:

The hourly rate for this position is $26.44-$27.40/hour with the opportunity for quarterly success payments and mileage reimbursement.
