Job summary

The Coordinating Center is seeking a Case Manager II for the Medical Day Care Services Waiver (MDCSW) program. You will provide person-centered case management and advocate for applicants and clients while adhering to COMAR regulations and Maryland MDH requirements.

The Case Manager II collaborates with the MDCSW Team to maximize outcomes and maintains documentation and billing compliance. Responsibilities include developing a POS, conducting care coordination via telephonic outreach and on-site

Qualifications

  • Experience as a Supports Planner or CCS is required at this time.
  • Experience working with medical based community resources is strongly preferred.
  • Bachelor's degree in human service focus psychology social work public health etc is strongly preferred

  • Develop a Person Centered Plan of Service for all clients on your caseload.
  • Coordinate care through telephonic outreach and on-site visits including medical day care programs and client homes.
  • Evaluate client needs using assessments and information sources; recommend supports and services.

What You Will Do

This position provides ongoing professional case management and advocacy work on behalf of applicants participants clients in the Medical Day Care Services Waiver MDCSW Program The Case Manager 2 Medical Day supports the mission of The Coordinating Center by providing person centered case management services while adhering to COMAR regulations the parameters of the Maryland Department of Health MDH Office of Long Term Services and Supports and The Centers policies The Case Manager 2 is responsible for effectively managing their time and resources to comply with specific contract requirements maintain database integrity and maintain documentation The Case Manager 2 works interdependently with the MDCSW Team to maximize quality outcomes for individuals As a Case Manager you will Develop a Person Centered Plan of Service POS for all clients on your caseloadConduct care coordination through telephonic outreach and on site visits including medical day care programs and client homesEvaluate client needs using assessments and various information sources; recommend supports and servicesMaintain consistent contact with clients and their designated support team to monitor well being and identify service needsCollaborate with community agencies and providers to connect clients to appropriate resourcesUtilize critical thinking skills to address service issues and client crisesEducate clients and providers on using technology to access and track servicesIdentify and report suspected abuse neglect or exploitation with supervisor supportSubmit Reportable Events to MDH upon notification of incidents or complaintsEducate individuals on advocacy resources and promote self advocacy and positive health behaviors to support client well beingEnsure compliance with all applicable COMAR regulations contract requirements and organizational policiesDocument coordination activity and resource information in multiple computerized systems including LTSSAdheres to billing and documentation guidelines including timely entry of case notes to meet minimum daily documentation and service hour requirementsRecommend improvements to programs procedures and service delivery effectivenessBuild and maintain relationships with long term care facilities statecounty agencies and law enforcement to support client needsManage own schedule to ensure timely completion of responsibilitiesAttend staff meetings in services and training sessions for skill developmentComplete assigned case management tasks and resolve concerns as neededTriage calls and inquiries from clients families providers and agencies to resolve issuesEnsure compliance with all relevant federal state and local regulationsActively participate in all assigned orientation training and precepting exercisesDemonstrate flexibility in responding to changing situations and reprioritizing workOrganize job functions and work area to effectively complete assignmentsMinimizes non productive time by engaging in activities that support future needs of The Center
