Job summary

Foundation Communities is seeking a Program Manager to lead the Community Support Services (CSS) program across multiple properties. The role ensures trauma-informed, person-centered case management, clinical services, and youth development are coordinated for resident stability and well-being.

The position oversees staff development, partnerships with providers, and data-driven outcomes, aligning with mission and equitable support. Some travel to diverse properties is required.

Qualifications

• Master’s degree in Social Work, Counseling, or a related field.
• Current LCSW-S, or LPC-S in the State of Texas.
• 3–5 years of experience in case management, behavioral health, or supportive housing, including supervisory or leadership responsibilities.

Responsibilities

• Provide leadership and supervision for the Community Support Services (CSS) Case Management team, including Clinical, Development, and General Case Managers.
• Provide clinical supervision towards independent licensure for LMSW and/or LPC-As, including one hour supervision weekly.
• Oversee daily program operations to ensure services are trauma-informed, equitable, efficient, and compliant with agency and funding standards.
• Develop and maintain policies, procedures, and workflows that support consistent, high-quality service delivery.
• Supervise, coach, and evaluate staff performance, offering reflective supervision and ongoing professional development opportunities.
• Foster a supportive, trauma-informed work environment that promotes staff well‑being, confidence, and resilience.
• Coordinate and monitor contracted therapeutic providers to ensure quality and alignment with program goals.
• Track and analyze program data to monitor outcomes, service utilization, and overall program effectiveness.
• Prepare reports that demonstrate impact, identify trends, and guide continuous quality improvement efforts.
• Collaborate with property management, Learning Center leadership, and other internal teams to ensure coordinated resident support.
• Build and sustain partnerships with behavioral health providers, community agencies, and local organizations to strengthen service networks.
• Represent Foundation Communities at meetings, coalitions, and community events related to resident services and behavioral health.
• Support community outreach efforts and participate in leadership meetings, trainings, and cross-departmental initiatives.
• Uphold Foundation Communities’ commitment to equity, inclusion, and trauma-informed care in all aspects of service delivery.
