Job summary

Envision Unlimited is a Chicago‑based nonprofit delivering comprehensive services to people with intellectual, developmental and psychiatric disabilities. The Program Coordinator supports members by coordinating all aspects of their program, including CDS center activities, community engagement, and maintaining files.

The role emphasizes advocacy, meeting participation (PCP, BMC, HRC), and ensuring services align with member goals while complying with funding requirements.

Qualifications

• Bachelor’s degree in Rehabilitation Counseling, Psychology, Social Work, Special Education, or related social service field preferred not required.
• One year of full-time related work experience.
• Must be able to meet qualifications of QIDP - (Have at least two years of verifiable experience working in Social Services.)
• Must be able to complete the requirements for placement on the State of Illinois Direct Support Persons Registry.

Responsibilities

• Ascertain member wishes regarding programming.
• Coordinate, supervise, and monitor staff and member activities to ensure that training programs and activities are correctly implemented.
• Participate in the Discovery process, PCP and Implementation Strategy. Support member advocacy to ensure the PCP/Implementation Strategy reflects the wishes of the member.
• Support member meetings including PCP, BMC, HRC and any special meetings; advocate for member wishes.
• Meet with members and invite parents/guardian, CLS staff, ISC or others to meetings.
• Assist members in obtaining/maintaining benefits, residential services, employment and advocacy services.
• Ensure programs/activities comply with funding source requirements and supervise staff planning.
• Ensure member access to the community is maximized, including accompanying them during community access.
• Hire, train, and evaluate staff; participate in staff hiring and ensure involvement of members.
• Respond to medical and behavioral emergencies following health & safety procedures.
• Monitor staff documentation and ensure supplies and personal care are met.
• Maintain, prepare, and submit periodic reports and records.
• Respond to emergencies, analyze problems, and train staff on procedures.
• Participate in site policy making and maintain safe work environment.
• Maintain effective relations with funding sources and agencies.
• Maintain current member habilitation files.
• Participate in staff training and continuing education.
• Ensure timely billing and reporting of attendance and Title XX activities.
• Maintain member records in reporting database.
• Coordinate special projects and other related responsibilities.

Education

Bachelor’s degree in Rehabilitation Counseling, Psychology, Social Work, Special Education, or related social service field preferred not required

One year of full-time related work experience

QIDP qualifications (two years of verifiable experience in Social Services)
