Job summary

TemPositions Group of Companies is seeking a Program Coordinator to support Day Hab services for individuals with intellectual and developmental disabilities in Staten Island, NY. The role emphasizes life planning, documentation, and compliance with OPWDD and Medicaid.

You will coordinate Life Plans, ensure accurate records in PrecisionCare/EHR, and collaborate with Care Managers, nursing, and interdisciplinary teams to promote person-centered outcomes.

Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology, Healthcare Administration, or related field.
• At least 2 years of experience working with individuals with I/DD.
• Experience with OPWDD regulations and documentation.
• Knowledge of person-centered planning and Medicaid requirements.
• Strong organizational, communication, and documentation skills.
• Experience with PrecisionCare/EHR and Life Plan preferred.

Responsibilities

• Coordinate Life Plans, Staff Action Plans, and person-centered services.
• Review and maintain program documentation for accuracy, completeness, and compliance.
• Monitor PrecisionCare/EHR records and ensure documentation supports Medicaid billing.
• Conduct quality assurance audits and track corrective actions.
• Ensure compliance with OPWDD, Medicaid, HIPAA, and agency requirements.
• Coordinate admissions, transfers, and discharges.
• Collaborate with Care Managers, nursing, clinical staff, families, and interdisciplinary teams.
• Participate in Life Plan, clinical, medical, and team meetings.
• Support audits, surveys, and corrective action plans.
• Identify documentation/workflow issues and recommend process improvements.
• Promote person-centered practices and ensure services align with individuals’ goals, preferences, and outcomes.
