Job summary

Envision Unlimited, a Chicago-based care organization, seeks a Program Coordinator to support members across the CDS center and community activities, maintaining member files and coordinating programming. The role includes PCP/Discovery support, advocacy, and collaboration with families and staff to meet member needs.

Qualifications include a Bachelor’s in a related field and at least 1 year of relevant experience; QIDP eligibility and Illinois Direct Support Registry requirements are valued.

Qualifications

• Bachelor’s degree in Rehabilitation Counseling, Psychology, Social Work, Special Education, or related social service field preferred not required.
• One year of full-time related work experience.
• Must meet qualifications of QIDP (2+ years verifiable experience in Social Services).
• Must complete requirements for placement on the State of Illinois Direct Support Persons Registry.

Responsibilities

• Ascertain member wishes regarding programming.
• Coordinate, supervise, and monitor staff and member activities to ensure training programs and activities are correctly implemented.
• Participate in the Discovery process, PCP and Implementation Strategy; support member advocacy to ensure the PCP/Implementation Strategy reflects member wishes.
• Support members in attending meetings (PCP, BMC, HRC) and advocate for their wishes at these meetings.
• Meet with members to involve parents/family/guardians and other stakeholders in meetings.
• Assist members in obtaining/maintaining benefits, residential services, employment, placement and advocacy services.
• Ensure programs comply with funding source requirements and supervise staff in planning/implementing activities.
• Maximize member access to the community, including accompanying members during outings.
• Hire, train, and evaluate staff; ensure member participation in hiring.
• Respond to medical and behavioral emergencies following health, safety and crisis procedures.
• Monitor documentation, staff plans, and supplies; meet members’ personal care and medical needs.
• Maintain and submit periodic reports and records related to members and activities.
• Respond to emergencies; analyze problems and train staff on procedures.
• Participate in site policy making and maintain safe working conditions.
• Maintain relationships with funding sources, referral agencies, and staff.
• Maintain current member habilitation files and participate in staff training.
• Ensure weekly/monthly billing and reporting of attendance and Title XX activities.
• Maintain member records in reporting database.
• Coordinate special projects as assigned.
• Perform other related responsibilities as assigned.

Education

Bachelor’s degree in Rehabilitation Counseling, Psychology, Social Work, Special Education, or related social service field

One year of full-time related work experience

QIDP qualifications (2+ years verifiable experience in Social Services)
