Job summary

Prairie Community Services seeks a Program Coordinator for Heartwood Home in Morris, MN, to oversee the 24‑hour operation serving individuals with disabilities and mental health needs. You’ll supervise staff, coordinate person-centered programming, maintain records, and ensure regulatory compliance.

Starting salary is $58,240. Responsibilities include developing services/outcomes, scheduling, training, and ensuring meals and medications are handled correctly, with a focus on safety and quality

Qualifications

• Must be 18 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license and meet company driving criteria.
• Must be able to read, write, comprehend and communicate in English.

Responsibilities

• Develop, implement, evaluate, update, and train on each person’s services, supports, and outcomes.
• Ensure the needs and program expectations of persons served are met 24 hours/day.
• Follow recipes, menus and diets as ordered when preparing and serving meals/snacks.
• Ensure medical treatments and medications are ordered, secured, stored, documented, and given as medically ordered.
• Communicate professionally and courteously with people we serve and their family members, outside services and providers, all PCS staff, and the public.
• Develop and maintain staff schedules including approving/denying staff requests for time off.
• Ensure documentation of daily program information.
• Attend and/or lead all in-services, develop and maintain staff schedules complete employee performance reviews, hire discipline and terminate staff.
