Job summary

Hands of Heartland is seeking a Program Coordinator to oversee the daily operations of clients' homes and the Individual Plans. You will supervise Direct Support Professionals (DSPs) and lead in implementing daily living activities with a focus on safety, dignity, and independence.

Qualifications

  • Two years of experience working with people with developmental disabilities preferred.
  • One year supervisory/leader experience preferred.
  • Valid driver's license.
  • Current automobile insurance
  • Ability to pass registry and background checks

Responsibilities

  • Ensure proper services are provided to address the wants and needs of people with developmental disabilities as written in the ISP.
  • Create a safe, positive, and encouraging atmosphere in the homes for the people we support.
  • Protect individuals' rights and treat them with dignity and respect.
  • Demonstrate and train team members to interact in an emotionally supportive and therapeutic manner.
  • Complete and maintain accurate documentation including ISP data, plans, logs and requests.
  • Safeguard individuals' funds and assist with purchases.
  • Ensure timecards are recorded and approved timely.
  • Provide necessary staff training and conduct trainings when authorized.
  • Perform regular and unannounced site visits and respond to incident reports.
  • Support community integration and assist with emergency on-call coverage.
  • Ensure facilities and vehicles meet requirements and stay compliant.

Job Summary

The Program Coordinator oversees the daily operations of assigned clients homes and the Individual Plans of these clients. Supervises Direct Support Professionals (DSP) working with Brighter Day clients in their home and community. Provides hands-on leadership in implementing and documenting clients daily living activities and assists in general operations of the individuals homes. Provides services which enables clients to attain and maintain the highest level of independence possible.

Essential Duties & Responsibilities
  • Ensure proper services are provided to address the wants and needs of people with developmental disabilities as written in the Individual Support Plan (ISP).
  • Ensure a safe, positive, and encouraging atmosphere in the homes for the people we support.
  • Ensure each individuals’ rights are protected and they are treated with dignity and respect.
  • Demonstrate and train team members to interact with the people we support in an emotionally supportive and therapeutic manner.
  • Ensure all documentation is completed accurately and in a timely manner,including but not limited toindividual plan data, behavior plan data, community integration logs, assessments, individual funds requests, communication book, mileage logs, maintenance requests forms, and supply acquisition forms.
  • Safeguard individual’s funds, assisting individuals with purchases.
  • Ensure assigned team members record their time accurately and approve timecards in a timely manner.
  • Ensures staff receive necessary training, both classroom and in-service. May conduct training when authorized.
  • Performs regular and unannounced site visits.
  • Reviews and responds to incident reports.
  • Responds to requests from outside agencies in a timely manner.
  • Facilitate the integration of individuals into the community by accompanying them to recreational and social activities.
  • Intervene when aggressive and/or inappropriate behavior occurs to implement behavior plans and strategies.
  • Provide on-call support to staff during emergencies and locate staff to fill shifts when necessary.
  • Ensure assigned facilities and vehicles are clean and compliant with Company and state requirements.
  • Complete and remain current on all required trainings.
  • Other duties as assigned.
Education & Experience
  • At least two years of experience working with people with developmental disabilities preferred
  • One year supervisory/leader experience preferred
  • Valid driver's license

  • Ability to pass registry and background checks

Physical Requirements
  • Ability to be on-call and potentially provide coverage of assigned facilities

  • Ability to use verbal and physical intervention to redirect and/or restrain a person who is physically aggressive or self‑abusive
  • Ability to safely provide supervision and monitoring to meet the needs of people supported by Hands of Heartland
  • Ability to perform repetitive tasks for extended periods of time to include but not limited to bending, stooping, sitting, and reaching
