CLASSIFICATIONS: This position is an administrative, full-time position. This position is salary andFLSA non-exempt.
REPORTS TO: Executive Director
PRIMARY PURPOSE: The primary duty is to oversee and manage the community-basedservices at Hope Haven: Community Access Group (CAG) Services for adults withdevelopmental disabilities. This position provides human services and casemanagement across all the community-based programs. This position isprincipally responsible for supervision, program compliance and management,development of persons served, volunteer and community integrationopportunities, ensuring that billing for services is maximized, communitynetworking, and being a liaison between persons served and staff and betweendirect support and administrative staff. This position oversees CAG Services byincreasing opportunities for adults with developmental disabilities toparticipate in activities both in the community and at the facility.
PRINCIPAL ACCOUNTABILITIES: This position is accountable to theindividuals that we support and their representatives, direct support staff,other administrative staff, and community partners.

ESSENTIALFUNCTIONS AND PHYSICAL REQUIREMENTS:

Musthave safe reliable transportation to get to work site as well as to transportindividuals to appointments and community activities
Ability/meansto communicate/document important information
Abilityto maneuver steps and other barriers to some homes

Abilityto perform CPR and First Aid
Abilityto perform Safety-Care Crisis Prevention Training Techniques
Abilityand willingness to lift up to 100 lbs. at least 4 times per day with adaptiveequipment (e.g., when providing personal care to individuals or moving contractwork)
Abilityand willingness to move fully loaded work pallets and skids using adaptiveequipment Ability and willingness to assist in lifting and transferring peopleup to 200 lbs.
Abilityand willingness to work in an open environment with noise, dirt, and fumes.
Abilityto hear, see, and communicate verbally within a level of basic understanding.
Casemanagement and record keeping experience
Serviceplan and program outcome development, implementation, and evaluation.

MINIMUMQUALIFICATIONS:
B.S.or B.A. in Psychology, Sociology, Social Work, Education, or closely relatedfield preferred, may be waived with experience
