We’re seeking a full-time Professional I, Behavioral Health Specialist/ Case Manager to provide support and advocacy for those who enroll in our FIRST team services. FIRST is a comprehensive treatment program designed to identify and treat clients experiencing a first episode of psychotic illness, specifically Schizophrenia, Schizoaffective Disorder, Schizophreniform Disorder, and Other Specified/Unspecified Schizophrenia Spectrum and Other Psychotic Disorders.
As a member of the FIRST Team, the Case Manager assists the client with accessing community resources, coordination and linkage to services, and developing the skills necessary to increase independence and functioning in the community, and also assists the client with educational needs.
The ideal candidate will have an Associates degree in social work, psychology, or a mental health-related field with 3 years of case management or care coordination experience, a valid driver’s license in the State of Ohio with access to an insured vehicle for client transportation, the ability and desire to work closely with other team members, and the ability to form effective therapeutic relationships to engage clients in the mutual assessment of problems and goal setting, and to help coordinate care plans from initial contact through end of service.
