You will perform counseling, case management, and treatment coordination with providers, while tracking progress and ensuring compliance.

The role requires a bachelor’s degree in social services or criminal justice and 2 years’ case management experience.

Qualifications

• Bachelor’s degree in social services or Criminal Justice with 2 years case management experience.
• Equivalent education and experience will be considered.
• All educational credits must be from an accredited institution.

Responsibilities

• Investigates and supervises high-risk individuals on probation or PSCs.
• Performs counseling, case management and reporting.
• Develops team supervision with referral sources.
• Participates in treatment processes and relapse planning.
• Facilitates groups and conducts cognitive programs.
• Prepares reports and updates client databases.
• Performs other duties as required.
